Main refactors are done — now I'll remove the legacy files and verify everything works.
Done. Ledger API renamed to `record`/`list`, `postEntry` kept only as `// keep` shim delegating to `record`, `getEntries` removed, all call sites updated, `lib/legacy.js` and `src/legacy-report.js` deleted. `node --test test/*.test.js` passes (2/2).
